Half Day Preschool Enrollment Form - You can download this form to begin the Half Day Preschool or Minds in Motion enrollment process. To complete the entire enrollment process, please contact the preschool office at (636) 851-6400 to check for Half Day or Minds in Motion preschool availability, then download and complete the enrollment form and bring it along with your child's immunization record and payment of the activity/supply fee and applicable tuition fees (cash or check) to the center. Please remember that you are NOT enrolled until you secure a spot and we receive your activity supply fee, applicable tuition payments and immunization records.

Full Day Preschool Enrollment Form - You can download this form to begin the Full Day Preschool enrollment process. To complete the entire enrollment process, please contact the preschool office at (636) 851-6400 to check for Full Day preschool availability, then download and complete the enrollment form and bring it along with your child's immunization record and payment of the activity/supply fee and applicable tuition fees (cash or check) to the center. Please remember that you are NOT enrolled until you secure a spot and we receive your activity supply fee, applicable tuition payments and immunization records.

Parents As Teachers Form - Parents As Teachers is a FREE program offered to all Francis Howell residents with children ages prenatal to pre-kindergarten. You can download this form, fill out the required information and email it back to us at margie.dieckman@fhsdschools.org or mail it to the address on our contact information page. The completed form may also be dropped off at any one of our three Early Childhood locations. Please remember that you must give us a current phone number and/or email address so that we are able to contact you to schedule in-home visits for your children who are prenatal to three years of age. We will also be able to email you the Early Childhood Newsletter which is sent out quarterly.

PALS Enrollment Form - You can download this form, fill out the information and return it to the center along with your membership fee (cash or check made out to PALS). Your PALS Enrollment form and membership fee can be dropped off at any Early Childhood location or can be mailed to the address on our contact information page. Please see the PALS Program tab on this website under FOR PARENTS for information on current membership fees.
